  • Firearm certificate application form (Form 101)
    This form should be used to apply for a firearms certificate and should be used in conjunction with the Referee form (Form 125).

  • Homophobic/Transphobic incident reporting form
    This form can be used to report homophobic or transphobic incidents. It can be filled in, printed off and returned to Leicestershire Constabulary, who will record the incident, investigate it and do their best to stop similar incidents happening in the future.

    This form can be used to make a note of any items of value you have, along with serial numbers and descriptions. It can be filled in, printed off and kept in a safe place. In the event of burglary where property is taken, this list may help identify your items if they are recovered by the police.
